Time to Register for Camp!
It's time for another exciting summer of fun, friendship, and learning at Camp Tuhsmeheta. Below you'll find all of the forms you'll need to register for camp.
Instructions
Step 1: Download and fill in the "Camper Registration", "Camper Health History", and "Photo Release" forms below.
Step 2: Email all completed forms to jackie@campt.org so we have an electronic copy of your application.
Step 3: If you wish to apply for a scholarship, read and complete the scholarship information found below.
Step 4: Finally, mail the signed forms with payment to:

We require a minimum $25 deposit or your scholarship forms at the time of application. You can pay the balance when you arrive at camp.

Family Camp
We'd love to see you and your family for the MPVI Family Camp, June 11-13. If you are only attending this weekend, just fill out the Family Camp Registration Form.

Scholarship Information
The Michigan Department of Education Low Incidence Outreach offers scholarships for low-income residents of Michigan.
Up to $1,200 is available per qualifying child per year. That's up to 4 camp sessions!
To apply, please complete each of the following and mail to P.O. Box 46, Green Ville, MI 48838. Or you can FAX them to (616) 754-0938.
- Scholarship Application - Complete one for each session of camp you wish to attend
- Scholarship Eligibility Certificate< - Must be completed by a TCVI, eye doctor, or other compitent authority/li>
- Proof of financial need: A copy of the school's free or reduced lunch form or a copy of a current Bridge card
